ELIGIBILITY FOR HOME DELIVERED MEALS

The following persons are eligible to participate in the Home Delivered Meal Program:

*Persons aged 60 or over who are unable to attend a Dining Center and may have one or more of the following limitations:

*lack of a support system to assist with meal preparation

*a physical, emotional, or cognitive impairment

*inadequate kitchen facilities which prevent them from preparing a nutritionally adequate meal.

Only Highland Valley Elder Services staff may determine eligibility. Therefore, refer each potential candidate to the Information and Referral Department at 586-2000 (or 1-800-322-0551). After eligibility is established, the Dining Center will be notified by email. This will be followed by a written authorization

A referral will be handled within three business days and an emergency referral within 24 hours.

Home delivered meals may not begin until authorized by Highland Valley. One exception to this is noted below.

The Dining Center Coordinator may authorize up to five consecutive days of home delivery to an enrolled Dining Center participant who is temporarily ill or incapacitated. These meals are to be ordered as congregate food and packaged at the Dining Center for home delivery. The Dining Center Coordinator must keep an adequate supply of trays and lids on had for this purpose. Meals beyond the fifth day must be authorized by Highland Valley. The Director should make the referral to the Information & Referral Department as necessary.

INFORMATION FOR NEW HOME DELIVERED MEAL PARTICIPANT

Each new participant is to receive the following during the first week of meal delivery:
“Home Delivered Meals Program Introductory Letter” (includes cancellation policy)
“Meal Contributions”
Contributions Envelopes
“Winter Storm Cancellation Sheet”
