ACCESS:

Assisted elders to find available services by:

 

Answering 237 calls for INFORMATION and REFERRAL.

 

Providing 72 units of Access Assistance to 34 caregivers.

 

Providing 117 HILLTOWN elders with 973 rides.

 

Providing DIRECT LEGAL REPRESENTATION to 3 persons (i.e. new cases).

IN-HOME:

We increased elders' in-home options by:

 

Providing CARE MANAGEMENT service to 1,070 people.

 

Providing 682 persons with 7,493 hours of HOMEMAKER service.

 

Providing 292 persons with 2,678 hours of PERSONAL CARE service.

 

Providing 6 elders with 47 hours of CHORE service.

 

Providing 5 elders with 14 days of SOCIAL DAY CARE service.

 

Providing the caregivers of 67 elders with RESPITE CARE service and 68

 

hours to 16 caregivers.

 

Providing 1,084 persons with 4,738 HOME DELIVERED MEALS.

 

Visiting 40 elders in the community for a total of 172 hours through the

 

SENIOR COMPANION program.

 

Reviewing and investigating 24 elder abuse intakes which resulted in 95

 

cases in investigation with 38 PROTECTIVE SERVICE open cases and 26

 

cases closed during the period.

COMMUNITY:

We increased elders' community options by:

 

Providing opportunities for 405 elders to get together over 3,175 MEALS at

 

our 11 DINING CENTERS for an average of 151 meals per day.

 

New referral of 2 potential host families and placement of 0 new AFC

 

clients. New client referrals 0 for AFC program. 20 current participants

 

with host families in the ADULT FAMILY CARE program.

 

Currently, 2 elders in the CARING HOMES program are receiving care by

 

family caregivers.

 

Paying 53 visits to elders in 12 nursing homes and rest homes through the

 

OMBUDSMAN program.

 

Receiving 104 COORDINATION OF CARE requests. Providing 63 nursing

 

home screenings during this period.

 

Assisted 46 elders with MONEY MANAGEMENT. Made 24 visits with

 

elders. 0 persons attended presentations.

 

Providing 5 units of Outreach activities to 118 elders.

 

Providing 15 units of Health Promotion to 19 elders.

 

Providing 1 units of Counseling, Support and Training to 4 elders.

 

4 Monitoring activities with 4 partners.
