RFP On-Line Application Procedures For Entering A New Form Completely |

| 1) | Once in your browser, please go to the site located at www.highlandvalley.org/RFP.html. |
| 2) | To submit an on-line application, please go to #9, under Application Instructions and click here. |
| 3) | Make sure that the option I want to enter a new form is selected. If so, please click Next. |
| 4) | Complete all fields. |
| 5) | At the end of the application, click To Confirmation Page >> . |
| 6) | You will receive a message that says Confirmation Is Being Processed. |
| 7) | Up in the left hand corner of the screen, there is an icon of a printer. Click on this, if you wish to print your on-line application. |
| 8) | Then, scroll all the way to the bottom and click Send. |
| 9) | The screen will indicate that it is Processing Form Entries...One Moment Please. |
| * Please note that if you are using Internet Explorer, then your browser will automatically close. | |
| * If you are using a different browser, such as Mozilla Firefox, then you will receive a screen with a message Powered by www.formdesk.com: | |
| 10) | Finally, you will need to notify John Lutz, Associate Director, via e-mail at jlutz@highlandvalley.org, that your on-line application has been submitted. |
| 11) | John will then send you a confirmation e-mail verifying that your application has been received. |
