our 15 DINING CENTERS for an average of 101 meals per day. |

Paying 47 visits to elders
in 12 nursing homes and rest homes through the |

OMBUDSMAN program. |

COORDINATION OF CARE requests. Providing 71 nursing |

home screenings during this period. |
|
Assisted 17 elders with
MONEY MANAGEMENT. Made 14 visits with |

elders. |

Outreach activities to 118 elders. |
