Assisted elders to find available service by:

Answering 303 calls for INFORMATION and REFERRAL.

Providing 40 units of Information and Assistance to 15 caregivers.

Providing 83 HILLTOWN elders with 708 rides.

Providing DIRECT LEGAL REPRESENTATION to 2 persons (i.e. new cases).

IN-HOME:

We increased elders' in-home options by:

Providing CARE MANAGEMENT service to 1,445 people.

Providing 623 persons with 7,371 hours of HOMEMAKER service.

Providing 284 persons with 2,359 hours of PERSONAL CARE service.

Providing 20 elders with 145 hours of CHORE service.

Providing 6 elders with 15 days of SOCIAL DAY CARE service.

Providing the caregivers of 54 elders with RESPITE CARE service.

Providing 1,084 persons with 4,600 HOME DELIVERED MEALS.

Visiting 41 elders in the community for a total of 227 hours through the SENIOR

COMPANION program and 30 hours to 7 elders.

Reviewing and investigating 36 elder abuse intakes which resulted in 26 cases in

investigation with 44 PROTECTIVE SERVICE open cases and 8 cases closed during

the period.

COMMUNITY:

We increased elders' community options by:

Providing opportunities for 435 elders to get together over 3,259 MEALS at our 17

DINING CENTERS for an average of 155 meals per day.

Currently, 2 elders in the CARING HOMES program are receiving care by family

caregivers.

Paying 61 visits to elders in 12 nursing homes and rest homes through the

OMBUDSMAN program.

Receiving 72 COORDINATION OF CARE requests. Providing 67 nursing home

screenings during this period.

Assisted 22 elders with MONEY MANAGEMENT. Made 29 visits with elders.

0 persons attended presentations.

Providing 6 units of Outreach activities to 2 elders.

Providing 8 units of Vision Rehabilitation to 10 elders.

Providing 119 units of Volunteer Development to 20 elders.

 

Providing 19 units of Home Repair to 14 elders.
Providing 625 units of Supportive Housing to 13 elders.

3 Monitoring activities with 3 partners.
